The Community-Based Care (CBC) Program, launched in 2021 as an alternative care model, has
demonstrated strong results in supporting highly vulnerable children through the efficient
use of minimal resources. The CBC approach is implemented through two primary interventions:
1. Family-Based Care (FBC):
This intervention offers continued support to children who were previously living in Rainbow
Homes and have now been reintegrated with their families after a thorough assessment and
preparation process. Both the child and the family undergo a structured transition to ensure
stability and well-being. Dedicated staff members carry out regular follow-up visits, providing
ongoing support and guidance to the children and their families.
